At Middle Flint Health & Wellness, our Adult Services are designed to provide comprehensive care and support for individuals facing mental health challenges, such as anxiety, depression, obsessive-compulsive disorder (OCD), and the effects of traumatic events.

Through education, counseling, skill-building, and community integration, we empower individuals to overcome challenges, achieve personal growth, and improve their overall quality of life. To address these needs, we provide the following programs:

  • Breezeway

    Peer Support and Psychosocial Rehabilitation (PSR) are the primary services provided. Other services include Behavioral Health Assessment, Service Plan Development, and Nursing Assessment and Care.

Peer Support is designed to provide persons served the opportunity to receive and provide support amongst and by people with lived experience and peers who have relatable issues. direct their own Recovery, learn, develop, and/or maintain skills needed to live independently in the community, obtain/maintain employment, develop natural supports, and other skills that will aid in the progression of the Recovery process and completion of individuals' Overall Rehabilitation Goal (ORG).

    Adults who have AXIS I Diagnosis

    Program Goals/Outcomes

    The program goals are to reduce the clients' reliance upon hospitalization, increase housing and job stability, and minimize suicidal, homicidal, and aggressive behavior. Decreasing the frequency of personal, social, community based problems is also a goal. The outcome measure is to provide all clients with at least the minimum number of face to face and collateral contacts per a 30 day period as well as have zero instances of inpatient psychiatric admissions at a state mental health facility (WCGRH).

  • Community Living Support (Personal Support Services)

    Community Living Support provides a learning environment, in which clients receive supervision and assistance in reaching individualized goals.

    Direct Services Offered

    Community Living Support provides a learning environment, in which clients receive supervision and assistance in reaching individualized goals, as well as:

    1. Supervision and assistance as needed with personal care.
    2. Planning and preparing nutritious meals and or snacks.
    3. Assistance in laundering and maintaining- purchasing appropriate clothing.
    4. Financial Management (i.e. development of monthly budget, paying monthly expenses).
    5. Recreational and leisure activities of choice.
    6. Transportation for medical appointments, activities, etc.
    7. Continuous assessment of needs and conditions.
    8. Training, if appropriate, to achieve goals of self-care and the possibility of moving into other levels of community placement.
    9. Increase family support in which stronger and healthier relationships are developed.

    Population Served

    Adults with Developmental Disabilities who meet the "Most-in-Need" criteria, live in (or have been referred to) the Middle Flint Health & Wellness service area, and admission criteria.

    Program Goals/Outcomes

    The goal of Family Support is to sustain and enhance the quality of family/home life so that the individual with developmental disabilities can remain within a nurturing family in his/her home. Support and service provision may be directed to the individual with the developmental disability or his/her family members.

  • Community Support Team (CST)

    Helping persons served live independently through advocacy and linkage to Community Resources.

    Services Offered

    The CST program is the most intensive level of case management services offered by Middle Flint Behavioral Health Care. The program is designed to provide individuals in the counties served Georgia APEX, School Based Mental Health Program was designed to provide mental health services to youth and adolescents within the school system to receive core mental health services within a school based setting.

    Population Served

    Individuals that meet criteria such as multiple psychiatric inpatients admissions or recent release from Georgia Regional State Hospitals and released from jail or prison.

  • Family Support (Personal Support Services)

    Direct Services Offered

    Community Living Support, Dental Services, Medical Care, Specialized Clothing, Recreation/Leisure Activities, Specialized Nutrition, Supplies, Homemaker Services, Transportation and any other support services.

    Population Served

    Individuals 3 years and older with Developmental and/or Intellectual Disabilities living within a family unit.

    Program Goals/Outcomes

    Reduce hospitalizations, increase community tenure, reduce disruptive behavior, and support job stability.

    Family Support services aims to:

    1. Keep families together until the individual with a disability chooses to live independently,
    2. Enhance a family's ability to meet the many needs of the family members with a disability,
    3. Improve the quality of supports to families while minimizing the need and cost of out-of-home placement to allow families to participate in recreational and social activities and,
    4. Make a positive difference in the life of the person with a disability and in the lives of all family members.

  • HIV Early Intervention Services

    Prevention: FREE HIV/AIDS Education; Rapid Testing and Counseling; and Linkage to care

    Population Served

    Our Consumers, their partners, staff and anyone in the community.

    Program Goals/Outcomes

    MFBHC program aims to reduce the transmission of HIV. We provided consumers and the community with information, demonstrations, referrals, and education directed towards informing drug users about the relationship between drugs (especially injection drug activity) and communicable diseases. Education on the impact of HIV/AIDS is done by equipping people with the necessary knowledge and skills to protect them from infection. We also encourage changes in high risk behavior, especially among peers and within the general community.

  • Intensive Case Management

    Population Served

    Adult individuals who have a severe and persistent mental illness that seriously interferes with their ability to live in the community as defined according to the ADA settlement as: "a diagnosable mental, behavioral, or emotional disorder of sufficient duration to meet diagnostic criteria that has occurred within the last year, has resulted in functional impairment which substantially interferes with or limits one or more major life activities, and has episodic, recurrent, or persistent features

    SERVICES OFFERED: The Resiliency Support Clubhouse program is designed to provide a comprehensive and unique set of services for children and families coping with the isolation, stigma, and other challenges associated with mental health disorders. The Clubhouse program is family-driven and youth-guided, culturally, and linguistically competent, and evidenced-informed., with the strengths and needs of the child and family determining the types and mix of services and supports.

    AGE GROUPS: 6 to 15 years of age. Age groups are broken into developmentally and clinically appropriate groups.

    Clubhouse program goals/objectives are to increase mental health functioning, improve school functioning, decrease DJJ involvement, increase youth engagement, increase community involvement, increase youth satisfaction, and increase community collaboration. Clubhouse Resiliency Support Services offers peer support services, education program (tutoring when available), clubhouse activities/social club (monthly outing), family involvement, transportation, nutrition, and mental health aftercare services.

Developmental Disability Services

We at Middle Flint Health & Wellness believe that persons with developmental disabilities also deserve a comfortable place to live, opportunities to socialize with others, productive time in a job or other meaningful activities, and the ability to function as independently as possible.

Intellectual or developmental disabilities include cerebral palsy, epilepsy, autism, or other neurological conditions. To address these issues, we provide the following programs:

  • Community Housing

    To provide personal support and skill training, develop and maintain skills in functional areas to enhance the quality of life for adults with developmental disabilities through increased participation in the community; develop opportunities for personal growth, develop and maintain social relationships, participate in social, recreational and community activities according to the individuals ISP. Quality of life outcomes include the following: improved health and wellness, reduction of unmet needs, life satisfaction related to the goals of the ISP.

    • MH Cluster Apartments

      Residential Housing in a group home setting, that provides group training in the implementation of daily living skills through home based activities in order to assist with enhancing self care skills.

      Adults who have MH AXIS I Diagnosis.

      Program Goals/Outcomes

      To integrate individuals into the community in the least intrusive environment that promotes the individuals housing choice, providing personal support and skill training to restore, develop, or maintain skills in functional areas in order to live meaningful lives in the community; develop or maintain social relationships; and participate in social, interpersonal, recreational or community activities according to the individuals IRP. Outcomes focus on: recovery, health and wellness, housing, employment and meaningful life in the community. Performance indicators are reduced hospitalizations and incarcerations; housing stability; participation in educational/vocational training or employment if desired; participation in community integration activities that promote recovery through social, spiritual, cultural and recreational activities

    Crisis Services

    At Middle Flint Health & Wellness, we are dedicated to providing immediate and compassionate support during times of crisis. Our Crisis Services are designed to help individuals and families navigate challenging situations, offering stability, guidance, and hope when it’s needed most.

    Whether facing mental health emergencies, trauma, or other urgent concerns, our team is available to provide professional care and connect individuals to essential resources. To address these needs, we provide the following programs:

    • Co-Responder Mobile Crisis Services

      Middle Flint Health & Wellness provides 24/7 mobile response provides immediate on-site crisis management through assessment, de-escalation, consultation, and referral with post crisis follow-up to assure linkage with recommended services. These services may be accessed by calling the toll-free Georgia Crisis & Access Line at 1-800-715-4225.

      Our mobile crisis team can provide an assessment by a licensed clinical professional to determine the most appropriate level of care. While some individuals may require an evaluation at an emergency receiving facility, others may be able to have their needs met through outpatient appointments by our providers.

      Mobile crisis engagement may be less traumatic for the individual that often results in linkage to our community-based services that provide the most appropriate level of care.

      What is a Mobile Crisis? A Mobile Crisis is intended to:

      • De-escalate crisis situations;
      • Relieve the immediate distress of individuals experiencing a crisis situation;
      • Reduce the risk of individuals in a crisis situation doing harm to themselves or others; and
      • Promote timely access to appropriate services for those who require ongoing mental health or co-occurring mental health and substance abuse services.

      Mobile crisis is a statewide service provided by DBHDD. This service is available 24/7/365 and can be requested through the Georgia Crisis and Access Line (GCAL).

    • Crisis Stabilization Unit

      Our Crisis Stabilization Unit (CSU) is dedicated to providing comprehensive crisis care services designed to support individuals in immediate need. Our CSU offers a range of essential services, including a crisis observation unit, a short-term psychiatric unit, a withdrawal management and detoxification unit, and a mobile crisis unit. Our mission is to guide individuals in crisis toward the most appropriate treatments while linking providers to create a seamless, integrated continuum of care—from prevention and crisis intervention through to rehabilitation. This approach ensures that you or your loved one receives the support and assistance necessary for recovery and well-being.

    At Middle Flint Health & Wellness, we believe that recovery is a journey of healing, growth, and renewal. Our Recovery Services are designed to support individuals overcoming substance use disorders and other challenges, helping them build healthy, fulfilling lives free from addiction.

    Through personalized care, evidence-based practices, and a supportive community, we empower individuals to reclaim their lives and sustain long-term recovery. To address these needs, we provide the following programs:

    • Cluster SA

      Direct Services Offered

      Semi-Independent Residential Housing that provides group training in the area of substance abuse through learning how to develop interpersonal and leisure activities in a home/community setting that will promote long term sobriety.

      Population Served

      Adults with addictive disease issues or co-occurring addictive diseases and mental illness diagnoses.

      Program Goals/Outcomes

      To integrate individuals into the community in the least intrusive environment that promotes the individuals housing choice, providing personal support and skill training to restore, develop, or maintain skills in functional areas in order to live meaningful lives in the community; develop or maintain social relationships; and participate in social, interpersonal, recreational or community activities without reliance on substance use, according to the individuals IRP. Outcomes focus on: addiction recovery/sobriety, health and wellness, housing, employment and meaningful life in the community. Performance indicators are reduced hospitalizations and incarcerations; housing stability; participation in educational/vocational training or employment if desired; participation in community integration activities that promote emotional wellbeing; health and wellness; and addiction recovery through social, spiritual, cultural and recreational activities

    • New Beginnings

      Direct Services Offered

      Population Served

      Adults who have AXIS I Diagnosis of Substance Abuse or Substance Dependence.

      Program Goals/Outcomes

      Emphasize reduction in the use and abuse of substances and/or continued abstinence; the negative consequences of substance abuse through addiction education.

      The program utilizes methods, materials, approaches, activities, setting, and appropriate outside sources to the developmental and cognitive levels, capabilities, age, and gender of participants.

      Program provides education on life style changes by learning to build recovery skills through changing environments, identifying triggers to substance abuse, developing coping techniques, learning to manage gender specific issues per consumer request, learning refusal/prevention skills, work skill development to enhance employability skills, and develop self responsibility.

    • New Life Addiction Recovery Resource Center

      Direct Services Offered

      non-clinical peer led, person-centered addiction recovery support, education, training, and advocacy grounded in "lived experience" that empowers and equips, while providing a safe and hope filled environment to those both in and seeking long-term recovery from the disease of addiction.

      Population Served

      Adults with addictive disease issues or co-occurring addictive diseases, Adults who have AXIS I Diagnosis of Substance Abuse or Substance Dependence.

Our Court Services are designed to offer support, accountability, and resources to help participants make positive changes in their lives and successfully fulfill legal obligations.

    Whether addressing substance abuse, anger management, or other behavioral concerns, we collaborate with courts and legal professionals to provide structured programs that promote rehabilitation and reduce recidivism. To address these needs, we provide the following programs:

    • Accountability Court Program

      Middle Flint Health & Wellness provides various levels of treatment services to adults with substance use disorders.

      The Accountability Court program is an integral part of Middle Flint Health & Wellness, offering crucial support and services to individuals who meet the criteria for both our programs and the Accountability Court initiative. The process begins with a referral from the court, initiating a comprehensive assessment by our court team to determine the individual's eligibility and needs.

      Our Court liaison team collaborates closely with the referring court and the service provider to understand the specific requirements of the individual. Through a thoughtful evaluation, we recommend appropriate services and ascertain if Middle Flint Human Health & Wellness can meet those needs. If a match is found, then individuals are enrolled in our services through the court process.

      In instances where our services may not fully align with the individual's needs, our dedicated team, in collaboration with attorneys and MFHW staff, actively seeks and facilitates alternative treatment options. We are committed to ensuring that every person receives the support and resources necessary for their rehabilitation and well-being.
