Program Goals/Outcomes
To integrate individuals into the community in the least intrusive environment that promotes the individuals housing choice, providing personal support and skill training to restore, develop, or maintain skills in functional areas in order to live meaningful lives in the community; develop or maintain social relationships; and participate in social, interpersonal, recreational or community activities according to the individuals IRP. Outcomes focus on: recovery, health and wellness, housing, employment and meaningful life in the community. Performance indicators are reduced hospitalizations and incarcerations; housing stability; participation in educational/vocational training or employment if desired; participation in community integration activities that promote recovery through social, spiritual, cultural and recreational activities
