Community Housing

DD Group Homes provide a learning environment, in which consumers receive supervision and assistance in reaching individualized goals, as well as:

    1. 24-hour protective care and watchful oversight.
    2. Supervision and assistance as needed with personal care.
    3. Three balanced, nutritious meals and two nutritious snacks per day.
    4. Supplies; such as linens, soap, etc.
    5. Assistance in laundering and maintaining-purchasing appropriate clothing.
    6. Recreational and leisure activities of choice.
    7. Transportation for medical appointments, activities, etc.
    8. Continuous assessment of needs and condition.
    9. Training, if appropriate, to achieve goals of self-care and the possibility of
    10. Moving into other levels of community placement.
    11. Increase family support in which stronger and healthier relationships are developed.
    Population Served

    Adult males and females with Developmental Disabilities who meet the “Most-in-Need” criteria, live in, or have been referred to, the Middle Flint Health & Wellness eight county areas and meet admission criteria.

    Program Goals/Outcomes

    To provide personal supports and skill training, develop and maintain skills in functional areas to enhance the quality of life for adults with developmental disabilities through increased participation in the community; develop opportunities for personal growth, develop and maintain social relationships, participate in social, recreational and community activities according to the individuals ISP. Quality of life outcomes include the following: improved health and wellness, reduction of unmet needs, life satisfaction related to the goals of the ISP.
