Program: Community Living Support (Personal Support Services)

Community Living Support provides a learning environment, in which clients receive supervision and assistance in reaching individualized goals.

Direct Services Offered

Community Living Support provides a learning environment, in which clients receive supervision and assistance in reaching individualized goals, as well as:

  1. Supervision and assistance as needed with personal care.
  2. Planning and preparing nutritious meals and or snacks.
  3. Assistance in laundering and maintaining- purchasing appropriate clothing.
  4. Financial Management (i.e. development of monthly budget, paying monthly expenses).
  5. Recreational and leisure activities of choice.
  6. Transportation for medical appointments, activities, etc.
  7. Continuous assessment of needs and conditions.
  8. Training, if appropriate, to achieve goals of self-care and the possibility of moving into other levels of community placement.
  9. Increase family support in which stronger and healthier relationships are developed.
Population Served

Adults with Developmental Disabilities who meet the “Most-in-Need” criteria, live in (or have been referred to) the Middle Flint Health & Wellness service area, and admission criteria.

Program Goals/Outcomes

The goal of Family Support is to sustain and enhance the quality of family/home life so that the individual with developmental disabilities can remain within a nurturing family in his/her home. Support and service provision may be directed to the individual with the developmental disability or his/her family members.
